To assist with your child's transition of care, we have identified two Children's Health Primary Care practices that you may wish to consider.


Our office utilizes the EPIC electronic medical record system through Children's Health. If you choose a provider who also uses EPIC, your child's medical information may be available through MyChart and Care Everywhere, which may help make your transition smoother.

If you choose another physician or clinic, we will be happy to assist with transferring your child's medical records.


While our office remains open, medical records requests should be submitted directly to our office. If your child establishes care with a new physician, the new provider may request your child's medical records after you complete and sign their medical records release authorization. Alternatively, our office can provide an Authorization for Release of Medical Records form upon request.

If you have questions or need assistance with transferring care or obtaining medical records, please contact a member of our office staff. We will be happy to assist you while our office remains open.
